HMM-263 was using forms pre-labelled for for HMM-263 until October 30, when they used pre-labelled forms for HMM-364 and HMM-165 and some forms did not have the old number scratched out and 263 written in (pages 1-20 of the PDF file are 30-31 October 1967).
